In the

- name of the event (if your event is not a part of a Calvary ministry, please describe it further)
- rooms requested
- date requested
- start/end time (include set up and tear down)
- equipment requested
- point person
- person in charge of unlocking/locking up doors
- person in charge of clean up

Your group will be responsible for set-up and clean-up for this event unless other arrangements are made in advance.
This will include:
- Throwing away all uneaten food items. No food is to be left on the counters or in the refrigerator.
- Taking out the trash to the dumpster and putting new liners in the trash cans.
- Sweeping the floors in the areas that were used.
- Washing, drying, and putting away all dishes that were used.
- Emptying and cleaning the coffee pots (if used).
- Cleaning the front doors (if needed).
- Leaving the building in the state it was found, if not better.
- Making sure all lights are turned off - restrooms and outdoor lights especially.
- Making sure all doors are completely shut and locked - side doors in the hallway especially.
